Social Determinants of Health
We understand there is a lot more involved in caring for your child(ren) than bringing them to their doctors’ appointments. Sometimes, families can encounter unfortunate circumstances, that can adversely impact children and their parents. In medicine, we call these Social Determinants of Health, and they include things like access to food, housing, and childcare.
Molina (Passport) Community Resources
- Passport has a special program to connect you with resources in your community. We can help you find housing resources, food and clothing locations and resources, transportation resources, support group information, applications for food stamps, Social Security, and utility bills, and much more. We also can help connect you to our Peer Support Specialists for Mental Health and Substance Use Disorder concerns, if you are having intimate partner violence issues, and much more.
