  • Sports Physicals

Getting your child a sports physical gives the doctor the opportunity to examine your child, evaluate their health and fitness, and determine whether or not they are prepared for sports or school activities.  Most of the time this can be completed at the same time as their annual wellness exam.  Please let us know if your child needs a sports form completed at the time of their annual wellness exam.
